Ways to Obtain a Death Certificate
1. A Death Certificate may be obtained by visiting any full service Virginia Department of Motor
Vehicles (DMV). Information regarding required documentation and forms can be found at
www.dmvnow.com. Go to the heading titled “Driver/ID Services” and then click on “Obtaining
a Virginia Vital Record”. A completed form titled “Virginia Vital Record Application (DL 82)”
must be presented to DMV together with the required documentation and due fee. The DL 82
form can be found by clicking on forms on the DMV website.
2. A Death Certificate may be obtained by mailing the completed “Virginia Vital Record
Application (DL 82)” to the following address: VDH Division of Vital Records PO Box 1000
Richmond, Virginia 23218. You must include a copy of your identification document and a check
or money order payable to the State Health Department in the amount of $12.00 with your
completed application.
