About This Booklet
This booklet contains information on all 50 states available for pharmacist licensure in 2016. The
following information is up to date as of June 30, 2017. The information in this booklet applies to
graduates of an ACPE-accredited College of Pharmacy who are applying for initial licensure by
examination in the United States of America. Graduates of a foreign school of pharmacy should check
online for information on how to apply for licensure. Additionally, applicants applying for licensure by
reciprocity or score transfer should check with the respective board of pharmacy to see if their
requirements differ. Please check the individual state’s licensing agency or board of pharmacy (BOP)
website for the most up to date information. ******We do not guarantee, endorse, promote or
support any of the programs listed in this resource guide it is only a list of information
we found at other sites and through other professionals.*******
Each state contains the following information:
- Link to the Board of Pharmacy website.
- Link to application form/online application, if applicable.
- Requirements and process to obtain license, including supporting documentation.
- Continuing education requirements, including details of type and applicable deadlines.
- Cost of initial licensure and other fees assessed.
- License renewal information, including renewal period and renewal fee.
- Pharmacy Law Resources, including links to that state’s pharmacy laws and statutes, as well as
information on live review sessions or study materials available, if applicable.
This information was updated and revised based on older documents put together by former and
current Butler University College of Pharmacy and Health Sciences pharmacy students:
Inderjit Ajimal, Jacquelyn Proffitt (graduates of BUCOPHS in 2013),
Brianna Patton (BUCOPHS graduate in 2014),
Caitlin Murphy (2015, BUCOPHS graduate),
Ryan Junkins (P4 pharmacy student, BUCOPHS, class of 2017), and
Anne Leighty (P4 pharmacy student, BUCOPHS, class of 2018).
Information on licensure was collected from the websites of state licensing agencies and boards of
pharmacy. Information on pharmacy law resources was collected by contacting pharmacy law
professors who are part of the American Society for Pharmacy Law (ASPL) educators list, as well as by
searching online for the most current state pharmacy laws. This booklet is in no way a
comprehensive review of all pharmacy law resources available.
