Alabama
Every person who desires to practice pharmacy within this state shall file with
the secretary of the board his or her written application for licensure upon forms
furnished by the board not less that 10 days prior to his or her examination.
The application shall be accompanied by an examination and registration fee for
residents and 13 nonresidents of this state, the fees to be set by the board.
The applicant shall furnish satisfactory proof that he or she is at least 19 years of
age, of good moral character, and that he or she holds a professional degree
from a division, school, college, or a university department of pharmacy
recognized by the State Board of Pharmacy.
Each applicant shall also be a citizen of the United States or, if not a citizen of the
United States, a person who is legally present in the United States with
appropriate documentation from the federal government.
The applicant shall have completed an approved practical training program
under the supervision of a licensed pharmacist in a site recognized by the board
as qualified for training pharmacy externs and interns, the training standards to
be established by the board as long as the standards are not less than those set
by the National Association of Boards of Pharmacy.
The applicant shall pass an examination administered by the board in subjects
consistent with those required by the National Association of Boards of
Pharmacy and in accordance with the rules and regulations of the board.

Alaska
An applicant for an Alaska Pharmacist license may apply for licensure by
examination or score transfer within twelve months after date of graduation
from an accredited college of pharmacy.
The following documents must be on file with the board to be considered for
licensure:
Completed, notarized application
Fees made payable to “State of Alaska”: $60.00 nonrefundable
application fee $240.00 license fee $20.00 wall certificate fee (optional)
Official transcript showing the degree granted sent directly to the division
from a college of pharmacy recognized by the Accreditation Counsel for
Pharmacy Education.
Verification of 1,500 hours of internship or experience in the practice of
pharmacy obtained in accordance with 12 AAC 52.080. Form enclosed.
Two affidavits from reputable citizens that the applicant has known for at
least one year attesting to the applicant’s good moral character
Notification from NABP of passing score on the NAPLEX exam
Proof of passing the MPJE. First, you must register with the NABP to take
the MPJE (www.nabp.net). After your application for an Alaska license is
complete, this office will notify the NABP that you are eligible to take the
exam. Within 2-3 working days, you will then receive an Authorization to
Test (ATT). Score Reports for the MPJE are available on the NABP and the
Alaska Board of Pharmacy websites.

Arizona
You must be a recent graduate of an accredited US university Pharmacist degree
program.
If you are a graduate of a foreign university, you must submit a copy of your
FPGEC certificate. You must also have earned 1500 intern hours in the US.
You must also be a US citizen or a legal resident with permission to work in the
United States and must meet all current US immigration policies. If you are here

Arkansas
Satisfactory proof of graduation and receipt of the first professional
undergraduate degree from a college of pharmacy approved by the Arkansas
State Board of Pharmacy; or Foreign Pharmacy Graduate Examination
Committee (FPGEC) Certification from NABP with 2,000 hours of practical
experience or training under Arkansas Board of Pharmacy approved conditions.
Applicants may request a blank application from the Board of Pharmacy, which
must be completed and returned to the Board of Pharmacy office together with
the fee as defined in regulation 01-00-0007. The application must be received no
later than the date designated by the Board for receipt of applications.
Each application must be accompanied by a recent 3" X 2" picture and a physical
description stating age, height, weight, color of hair, eyes, and complexion of the
applicant.
Each applicant must undergo a state and federal criminal background check
pursuant to Regulation 11, to be conducted by the Arkansas State Police and the
Federal Bureau of Investigation. The Board will furnish the forms and
instructions to applicants for the criminal background check. The applicant is
responsible for the payment of fees for criminal background checks pursuant to
written instructions provided by the Board.
The test or tests shall be graded and reported, and a reported score of 75 or
above is considered passing.
No person except members of the Board of Pharmacy or their authorized
representatives will be permitted to enter the testing site during the course of
examination.
The applicant must make a score of 70% or more on the jurisprudence exam
prior to making application for licensure as a pharmacist in the state of Arkansas.

California
Is at least 18 years of age.
Has graduated from a college of pharmacy or department of pharmacy of a
university recognized by the board
If the applicant graduated from a foreign pharmacy school, the foreign
educated applicant has been certified by the Foreign Pharmacy Graduate
Examination Committee.
Has completed at least 150 semester units of collegiate study in the United
States, or the equivalent thereof in a foreign country. No less than 90 of those
semester units shall have been completed while in resident attendance at a
school or college of pharmacy.
Has earned at least a baccalaureate degree in a course of study devoted to the
practice of pharmacy.
Has completed 1,500 hours of pharmacy practice experience or the equivalent in
accordance with Section 4209.
Has passed the North American Pharmacist Licensure Examination and the
California Practice Standards and Jurisprudence Examination for Pharmacists on
or after January 1, 2004.
Proof of the qualifications of an applicant for licensure as a pharmacist shall be
made to the satisfaction of the board and shall be substantiated by affidavits or
other evidence as may be required by the board.
Each person, upon application for licensure as a pharmacist under this chapter,
shall pay to the executive officer of the board the fees provided by this chapter.
The fees shall be compensation to the board for investigation or examination of
the applicant.

Colorado
Every applicant for a license under this article must read and write the English
language, or if the applicant is a partnership, each member of the partnership
must read and write the English language. If the applicant is a Colorado
corporation, the corporation must be in good standing, and if the applicant is a
foreign corporation, it must be qualified to do business in this state.
The board shall issue the appropriate registration to each manufacturer
and wholesaler that meets the requirements of this article unless the
board determines that the issuance of the registration would be
inconsistent with the public interest. In determining the Pharmacists,
Pharmacy, and Pharmaceuticals public interest, the board shall consider
the following factors:
Maintenance of effective controls against diversion of controlled
substances into illegitimate medical, scientific, or industrial channels
Compliance with applicable state and local laws
Any conviction of the applicant under any federal or state law relating to
a controlled substance
Past experience in the manufacture or distribution of controlled
substances and the existence in the applicant's establishment of effective
controls against diversion
Any false or fraudulent information in an application filed under this part

Suspension or revocation of the applicant's federal registration to
manufacture, distribute, or dispense a controlled substance as authorized
by federal law
Any other factors relevant to and consistent with the public peace,
health, and safety.
Every applicant for a license or registration under this article shall make written
application in the manner and form prescribed by the board, setting forth the
applicant's name and address, the applicant's qualifications for the license or
registration, and other information required by the board. The applicant shall
submit with the application the required fee, and, if the applicant is required to
take an examination, the applicant shall appear for examination at the time and
place fixed by the board.
An applicant who has graduated from a school or college of pharmacy approved
by the board may take an examination before the board.
The examination must be designed fairly to test the applicant's
knowledge of pharmacy and other related subjects and must be in a form
approved by the board. The examination cannot be administered orally.
An applicant for licensure by examination shall have completed an
internship as prescribed by the board.
A person who produces evidence satisfactory to the board that the
person has graduated and obtained a degree from a school of pharmacy
outside the United States and has passed a foreign graduate equivalency
test given or approved by the board may apply to take the examination
set forth in paragraph (a) of this subsection (4).
Every applicant for licensure as a pharmacist, whether by examination, transfer
of license, reactivation, or reinstatement, shall take a jurisprudence examination
approved by the board that tests such applicant's knowledge of the laws of this
state.
No applicant shall exercise the privileges of licensure or registration until the
board grants the license or registration.
The board may require any applicant for licensure to display written or oral
competency in English. The board may utilize a standardized test to determine
language proficiency.
A person licensed by examination and in good standing in another state may
apply for a license transfer. The board shall designate a clearinghouse for license
transfer applicants, and a person applying for a license transfer shall apply
through the clearinghouse designated by the board.
The board shall adopt rules as necessary to ensure that any person who
manufactures drugs and any wholesaler of drugs possesses the minimum
qualifications required for wholesale drug distributors pursuant to the federal
"Prescription Drug Marketing Act of 1987", 21 U.S.C. sec. 353, as amended.
A person whose license has been revoked shall not reapply for licensure earlier
than two years after the effective date of the revocation.
Issuance of a license or registration under this section and section 12-42.5-117
does not entitle a licensee or registered facility or outlet to wholesale,
manufacture, distribute, dispense, or professionally use controlled substances
beyond the scope of his or her federal registration.

Delaware
Whether you are applying by Examination or Score Transfer, you must complete
1,500 hours of Board-approved pre-licensure experience before you can be
considered for licensure. If you have not completed all 1,500 hours when you file
this application and you wish to attain any remaining hours while working in
Delaware, you must file an Application for Registration for Internship in addition
to this application. The Board office will issue you a permit to allow you to work
as a Pharmacist Intern in a Delaware Pharmacy.
The following are required for all applications, regardless of whether you are
applying by Examination or Score Transfer.
Submit completed, signed and notarized Application for Pharmacist
Licensure by Examination or Score Transfer.
Enclose non-refundable processing fee by check or money order made
payable to “State of Delaware.”
Arrange for Board office to receive a State of Delaware and Federal
Bureau of Investigation criminal background check following the
instructions on the Instructions for Requesting a Criminal Background
Check form included with this application. It is strongly suggested that
you submit your request to the State Bureau of Identification promptly
because it may take up to eight weeks for the Board office to receive the
criminal background check.
Submit proof of your pharmacy education:
If you graduated from a school or college of Pharmacy in the U.S.,
submit a Certificate of Graduation Form completed by your school
or college of Pharmacy and sent directly to the Board office.
If you graduated from a school or college of Pharmacy outside the
U.S., submit a copy of your Foreign Pharmacy Graduate
Examination Committee (FPGEC) Certificate. For an application, go
to www.nabp.pharmacy.
Arrange for the Board office to receive proof that you have completed
1,500 hours of Board-approved internship. Proof consists of any of these
that apply:
College Practical Experience Form completed by your school or
college of Pharmacy for practicum hours and sent directly to the
Board office
Affidavit of Intern Experience form(s) to document any pre-
licensure or internship hours you have worked in a Pharmacy,
completed by your preceptor and sent directly to the Board office.
Transfer of internship hours from another jurisdiction(s), sent
directly from each jurisdiction’s Board of Pharmacy to the
Delaware Board office
If you have never been issued a U.S. Social Security Number (SSN), submit
a Request for Exemption from Social Security Number Requirement .
In addition to filing this application, you must register with NABP to take the
Multi-State Pharmacy Jurisprudence Examination (MPJE) and, if you have not
already passed it, the NAPLEX. To register, go to www.nabp.pharmacy. You
should register with NABP at the same time you file this application. Until you
register with NABP, the Board office cannot make you eligible to take the exam.
The Board office will determine if you are eligible to take the examination when
it has received all of the following:

If you have graduated, the Board must receive either the
Certificate of Graduation or FPGEC, whichever applies.
If you have not yet graduated, arrange for the Board office to

letter must be sent directly from the school or college to the
Board office. This letter is sufficient only to allow you to take the
examination; it is not sufficient proof of education for licensure.
When the Board office makes you eligible, NABP will send you an Authorization
to Test letter that will explain how to schedule your examination.
NABP will notify you of the results. If you pass, all items listed in the
Requirements for All Applications section above will be required before your
Pharmacist license is issued.

Florida
In order to be licensed as a pharmacist in the State of Florida, you must apply to
the Florida Board of Pharmacy, and have passing scores on the North American
Pharmacist Licensure Examination (NAPLEX) and the Multistate Pharmacy
Jurisprudence Examination (MPJE) (also referred to as the “Florida law exam”).
The board is a participant in the NAPLEX Score Transfer Program. If you elect to
transfer your NAPLEX score to Florida, the score is good for three (3) years from
the date you took the examination.*If you passed the NAPLEX examination,
please review the requirements for licensure by endorsement to see if you
qualify by this method. If you would like to apply by endorsement, please
download an endorsement application.
You are required to submit Form A to certify your graduation, and document the
completion of 2080 hours of intern or work experience by submitting an
Internship or Work Experience Form. These hours may be sent in by one or all of
the following:
From the College of Pharmacy from which you received your degree
From the state board of pharmacy in the state you completed your
internship
From your Employer
All applicants must complete a Florida Board approved course on medication
errors prior to licensure. The course shall be no less than two (2) contact hours
and shall cover the subjects listed in Rule 64B16-26.103(1) (c), F.A.C. (Submit a
copy of the course completion certificate to the Board of Pharmacy.) Florida

Hawaii
Is at least 18 years of age and is a United States citizen, a United States national,
or an alien authorized to work in the United States.
Graduation from pharmacy school or college accredited or has received
candidate status by the American Council Pharmaceutical Education ("ACPE").
Pass the North American Pharmacist Licensure Examination (NAPLEX) and the
Multistate Pharmacy Jurisprudence Examination (MPJE) for this State, each with
a minimum passing score of 75.
All applicants for a Hawaii pharmacist license shall submit verification of at
least 1,500 hours of practical experience in a pharmacy. You may submit this
verification before or after you sit for the examinations.
Complete and submit an "Application for Exam and License - Pharmacist" with a
$50 non-refundable application fee made payable to "Commerce & Consumer
Affairs".

Illinois
Supporting Document CCA must be completed and submitted with each

School of Pharmacy from which your Pharmacy Degree was received and have
school seal affixed.
If you will be completing the NAPLEX in another state or jurisdiction and plan to

the date of the examination.
If you have been licensed as a pharmacist for less than one year, supporting

Iowa
Graduation from a recognized college of pharmacy or FPGEC certification.
The licensure exam consists of two components: North American Pharmacist
Licensure Examination (NAPLEX) and the Multistate Pharmacy Jurisprudence
Examination (MPJE). To be eligible for a license by examination, the candidate
must pass both components (with a total scaled score of no less than 75) within
a period of one year beginning with the date the candidate passed an initial
component.
Proof of 1500 hours of internship must be filed with the Iowa Board. If your
internship was obtained in a state other than Iowa, you must submit certification
from the Board in the state in which you obtained the 1500 hours.

Kansas
The application requires a $40.00 check or money order made payable to the
Board of Pharmacy as well as an additional $47 plus a completed Fingerprint
Card and S-100 Background Check form if you have not been fingerprinted for
the Kansas Board of Pharmacy previously
You will need to attach a copy of your driver’s license, a head & shoulders photo

Louisiana
To obtain a license to engage in the practice of pharmacy, an applicant for
licensure by examination shall:
(1) Be at least twenty-one years of age.
(2) Be of good moral character and temperate habits.
(3) Meet the following educational requirements:
(a) Have graduated and received a professional degree from an approved
college of pharmacy.
(4) Supply proof, substantiated by proper affidavits, of a minimum of one year of
service and experience through an internship or other program that has been
approved by the board under the supervision of a licensed pharmacist, which
service shall be predominantly related to the provision of pharmacy primary care
and the dispensing of drugs and medical supplies, the compounding of
prescriptions, and the keeping of records and the making of reports as required
under state and federal law.
The Board requires documentation of at least 1,500 hours of pre-
licensure practical experience. As evidence of the Board’s value
assessment of the professional experience program provided by ACPE
accredited schools of pharmacy, the Board will issue a 1,000 hour credit
for the successful completion of that program as demonstrated on the
Certification of Graduation. Therefore, the applicant must earn at least
500 hours in a licensed pharmacy separate and apart from any academic
program.
(7) Have passed an examination or examinations required by the board.
The applicant shall demonstrate compliance with the examination
requirement with proof of successful completion of the North American
Pharmacist Licensure Examination (NAPLEX) and the Multistate Pharmacy
Jurisprudence Examination for Louisiana (MPJE-La).

Maine
Examinations Effective May 1, 2017 the Maine Board will not longer be
qualifying candidates for the NAPLEX or MPJE. Prior to submitting this
application you must have obtained a passing score on both the NAPLEX and
Maine MPJE examinations. If you have passed the NAPLEX in another state, you
must contact NABP for a score transfer to Maine.

Massachusetts
In order to be registered as a pharmacist by examination by the Board, an
applicant must meet the requirements set forth in 247 CMR 3.01(1) or (2).
(1) Graduates of ACPE-accredited and Board-approved colleges/schools
of pharmacy:
(a)An applicant shall be eligible for examination for personal
registration as a pharmacist provided the applicant:
1. is 18 years old by the scheduled date of the examination
applied for;
2. has earned a qualifying degree in pharmacy from a
college/school of pharmacy accredited by the ACPE or approved
by the Board;
3. has acquired no less than 1500 hours of practical experience as
a pharmacy intern under the supervision of a Board-approved
pharmacist preceptor, of which at least 1000 hours must be
completed in a pharmacy or pharmacy related setting, as set forth
in 247 CMR 8.01; and
4. is of good moral character.
(b) An applicant shall properly apply to take NAPLEX and MPJE.
To qualify for personal registration, the applicant must
achieve a NAPLEX score of not less than 75% and an MPJE
score of not less than 75%.

Michigan
Michigan requires 1,600 hours of internship (including externship). The
Internship Training Affidavit form should be used only to report those intern
hours gained in Michigan while holding an intern license. Hours gained in other
states must be reported directly to this office by the Board of Pharmacy in the
state where the intern hours were obtained.
Graduates of ACPE approved pharmacy education programs must forward the
Certificate of Pharmacy Education form to your school of pharmacy to request

Minnesota
To be entitled to examination by the Board as a pharmacist the applicant shall be

college of pharmacy of the University of Minnesota or of a college or school of

Minnesota requires 1600 hours of internship experience worked after

Missouri
To be licensed as a Missouri pharmacist, applicants must:

Montana
The board has selected the National Association of Boards of Pharmacy (NABP)
licensure examination (NAPLEX) to be administered to candidates for licensure in
Montana. The examination shall be prepared to measure the competence of the
applicant to engage in the practice of pharmacy. A score of 75 shall be a passing
score for this examination. A candidate who does not attain this score may
retake the examination after a 90-day waiting period from the date of the exam.
In addition, the NABP shall administer a multistate pharmacy jurisprudence
examination (MPJE). This examination shall be prepared to measure the
competence of the applicant regarding the statutes and rules governing the
practice of pharmacy. A score of not less than 75 shall be a passing score for this
examination. A candidate who does not attain this score may retake the
examination after a 30-day waiting period from the date of the exam.

Nebraska
Pharmacist license requirements:
(1) To be eligible to take the pharmacist licensure examination, every applicant
must present proof of graduation from an accredited pharmacy program. A
graduate of a pharmacy program located outside of the United States and which
is not accredited shall be deemed to have satisfied the requirement of being a
graduate of an accredited pharmacy program upon providing evidence
satisfactory to the department, with the recommendation of the board, of
graduation from such foreign pharmacy program and upon successfully passing
an equivalency examination approved by the board.
(2) Every applicant for licensure as a pharmacist shall (a) pass a pharmacist
licensure examination approved by the board, (b) have graduated from a
pharmacy program pursuant to subsection (1) of this section, and (c) present
proof satisfactory to the department, with the recommendation of the board,
that he or she has met one of the following requirements to demonstrate his or
her current competency:
(i) Within the last three years, has passed a pharmacist licensure
examination approved by the board;
(ii) has been in the active practice of the profession of pharmacy in
another state, territory, or the District of Columbia for at least one year
within the three years immediately preceding the application for
licensure;
(iii) has become board certified in a specialty recognized by the Board of
Pharmacy Specialties or its successor within the seven years immediately
preceding the application for licensure;
(iv) is duly licensed as a pharmacist in some other state, territory, or the
District of Columbia in which, under like conditions, licensure as a
pharmacist is granted in this state; or
(v) has completed continuing competency in pharmacy that is approved
by the Board of Pharmacy.
(3) Proof of the qualifications for licensure prescribed in this section shall be
made to the satisfaction of the department, with the recommendation of the
board. Graduation from an accredited pharmacy program shall be certified by
the appropriate school, college, or university authority by the issuance of the
degree granted to a graduate of such school, college, or university.

A Nevada pharmacist’s license will not be issued until you have successfully
passed the NAPLEX and MPJE exams and submitted the following:
1740 Intern Hours (minimum). Verification of intern hours must come
directly to us from the state board of pharmacy where you are licensed
as an intern. We will also accept a verification of hours from your school.
NO EXCEPTIONS. INTERN HOURS ARE NOT REQUIRED TO TAKE THE

Documentation of completion of internship hours (1,500 hours are required for

New Jersey
13:39-2.1 REQUIREMENTS FOR INITIAL LICENSURE AS A PHARMACIST
An applicant for initial licensure as a pharmacist in New Jersey shall
satisfy the following requirements:
1) The applicant shall have graduated with either a degree of Bachelor of
Science in pharmacy with a minimum five-year course of study, or with a
Doctor of Pharmacy, from a school or college of pharmacy accredited by
the American Council of Pharmaceutical Education (ACPE) or deemed
ACPE-equivalent by ACPE;
i) The applicant shall submit an official transcript from the
registrar of the school or college of pharmacy substantiating that
the applicant has graduated;
2) The applicant shall have passed the North American Pharmacist
Licensure Examination (NAPLEX) and the Multistate Jurisprudence
Pharmacy Examination (MJPE), consistent with the requirements of
N.J.A.C. 13:39-2.2. The applicant shall take the NAPLEX and the MJPE only
after providing the Board with an official transcript and receiving
authorization to test from the National Association of Boards of
Pharmacy (NABP).
An applicant who has already taken the NAPLEX and has had his
or her scores transferred to New Jersey within five years of having
passed the examination consistent with N.J.A.C. 13:39-2.2, shall
take the MJPE only after providing the Board with an official
transcript and receiving authorization to test from NABP allowing
the applicant to be admitted to the MJPE examination;
3) If the applicant is applying for initial licensure more than two years
following his or her graduation from pharmacy school, the applicant shall
complete 1,440 hours of practical experience in a Board-approved
internship. The applicant shall register with the Board as an intern and
shall satisfy all internship requirements set forth in N.J.A.C. 13:39-2.6
within the two-year period immediately preceding the date of
application; and
4) The applicant shall have satisfied the good moral character and
criminal history background check requirements set forth in N.J.A.C.
13:39-2.3 and 2.4.
13:39-2.2 LICENSURE EXAMINATION SCORES a) An applicant for initial licensure
shall attain a passing score of not less than 75 on the North American Pharmacist
Licensure Examination (NAPLEX). If an applicant fails the NAPLEX, he or she shall
be required to repeat the examination.

New Mexico
Applicants for licensure by examination shall pass the standard national
examination currently known as the North American pharmacist licensure
examination (NAPLEX) and pass the multi-state pharmacy jurisprudence
examination (MPJE).
(1) In order to sit for the examination based on NAPLEX subjects, the applicant
must be a graduate from a college of pharmacy accredited by the American

(3) To pass the MPJE in the state of New Mexico, the candidate shall have a score
of at least 75.
(4) Candidates taking NAPLEX in participating states may transfer scores in
compliance with national association of board of pharmacy (NABP) “score
transfer program”, upon payment of the fee to the New Mexico board of
pharmacy for licensure by examination, and in compliance with such other
requirements set by the New Mexico board of pharmacy so long as New Mexico
participates in the NABP “score transfer program”.

received their score may take the MPJE.

In order to become licensed as a pharmacist in North Carolina the law requires

North Dakota Pharmacist Licensure Steps
Step 1: Register to take the NAPLEX/MPJE exams. You must receive a passing
score of 75 on both exams to be licensed in North Dakota.
Step 2: Notify the North Dakota Board of Pharmacy for licensure by examination
by completing and submitting an “Application for Licensure by Examination”
form. The form notifies the ND Board of Pharmacy your intention to obtain ND
licensure. The exam schedule will be MAILED to the applicant.
Step 3: The direct link to the “Application for Licensure by Examination” form is:

Ohio
EVERY APPLICANT FOR PHARMACIST LICENSURE IN OHIO, BY EXAMINATION,
SHALL:
Be at least eighteen (18) years of age.
Be of good moral character and habits.
Have obtained a degree in Pharmacy from a school of pharmacy program
approved by the Board in the year that the applicant graduated.
Have satisfactorily completed at least the minimum requirements for
pharmacy internship as outlined by the Board.
If the applicant successfully graduated after December 31, 2006,
with a doctor of pharmacy degree from a school of pharmacy
approved by A.C.P.E. and the State of Ohio Board of Pharmacy,
the internship credit requirement shall be deemed satisfactorily
completed.
If the applicant graduated on or before December 31, 2006 from
an approved school of pharmacy or from a foreign school of
pharmacy, the applicant must obtain at least 1,500 hours of
approved internship following licensure as an intern in Ohio or
any other state where the credentials are at least the equivalent
of those required by the Ohio Board at the time the experience
was obtained.

Applying For the Licensure Examinations
Candidates must complete and submit (with required attachments) the
"Application For Examination As A Pharmacist" for Ohio.
Candidates must apply online with NABP at www.nabp.net and pay the
required fees to register for NAPLEX and MPJE.
Once the Board office determines that a candidate is qualified to sit for
the Licensure Examination in Ohio and certifies that candidate to NABP,
the candidate will be sent an “Authorization to Test” (ATT) from the
testing center.
The State of Ohio Board of Pharmacy will mail the results to the
candidate only after receiving BOTH the NAPLEX and MPJE scores from
NABP and the Criminal Records Check
If an applicant fails the licensure examination, he/she will receive notice
to that effect. Those taking one or both of the examinations again MUST
file the application forms and pay the required fees AGAIN. Affidavits for
internship experience and the Certificate of Pharmacy Education need
not be submitted for subsequent exam(s).
Candidates who fail must wait a minimum of ninety-one days to retake
the NAPLEX and a minimum of thirty days to retake the MPJE per NABP
policy. This waiting period begins from the date the exam is taken, not
the date scores are mailed by the Board or received by the candidate.
Oklahoma
Licensure
All candidates must be a graduate of an accredited school or college of
pharmacy approved by the Board.
Foreign pharmacy graduates from a school or college of pharmacy not
approved by the Board shall meet the Board requirements for foreign
pharmacy graduates.
All applicants must pass a Board approved jurisprudence examination
and/or any licensure examination required by the Board including, but
not limited to, the NAPLEX®.
All applicants must verify their lawful presence in the United States.
All applicants may be required to appear before the Board for an
interview.

Completion of a first professional degree program in pharmacy located within
the United States, which is accredited by the American Council on

the experiential learning pharmacy practice course. To be eligible for licensure in

under the supervision of a licensed pharmacist. Each preceptor under whom

that board to forward a Verification of Internship Hours directly to the Rhode
Island Board.

Tennessee
For applying for initial pharmacy licensure, the Department of Health has been
working on an online application process that will allow all health care
professionals to apply online for an initial license and complete (and update as
necessary) a practitioner profile mandatory for certain professions. This system
allows the user to pay all fees and upload many of the documents required to
complete your initial application using the online portal.
All application fees are non-refundable.
Applicants must be at least 21 years of age, and be a graduate of an accredited
college of pharmacy.
The Tennessee Board of Pharmacy interprets the term “graduated” to
mean that the student has completed the actual graduation process and
has received a diploma.
A minimum of 1,700 (seventeen hundred) pharmaceutical internship hours are
required for licensure.
Applicants must submit affidavits attesting to internship hours acquired
in Tennessee with the application.
