1. Complete this form and attach a letter explaining your interest in one of the 4
areas of study listed above. In your letter discuss your training, experience,
accomplishments, volunteer work and employment experiences and how these will
figure in your future education and work .
2. Send 4 copies EACH of your completed application, high school transcript and
letters of recommendation [at least 3] to the Woman’s Club of Riverhead. The
application must be received no later than March 30, 2024. Use the PO Box
address provided above.
3. Notice of receipt of your application will be sent by e-mail. The e-mail will contain
interview details for either Saturday, April 6th or Saturday, April 13th.
Applicants must be available for both dates.
4. Winners will be notified no later than Monday, April 29, 2024 Awards shall be
presented at the Annual Scholarship Luncheon at Vineyard Caterers in Aquebogue,
May 15, 2024.

Interview Information
Each candidate will be interviewed by a panel of 3 persons.
The visual and performing arts students must bring in a portfolio of at least 3 examples of their
works; the performing arts candidates must be prepared with 2 pieces and are responsible for
brining their own music/instrument if needed.
Failure to keep the interview appointment results in elimination from further consideration.
